Veterans Outreach Counselor

Dudley, MA

The basic function of the Veteran's Outreach  Counselor is to assist Veterans and their families in making informed choices about setting, services, and financial resources that best meet their identified needs.  Benefits counseling and outreach may be provided in private homes and other community settings.  

Responsibilities:

  • Participate in training and orientation sessions as assigned. 
  • Provide the following services to Veterans and their families:
    • Unbiased information about support services available and methods of payment
    • A person-centered approach that ensures the Veteran’s goals and preferences are honored
    • Benefits counseling in a timely and appropriate manner based on the situation of the Veteran and their family
    • Identification of next steps
    • Assistance in facilitating referrals and resources as needed
    • 30-day follow up contact to ensure success
  • Coordinate activities with other Veterans Service Officers and veterans programming throughout the Tri-Valley service area.
  • Coordinate activities with other trained agency staff providing support services, including but not limited to:  I&R Workers, Care Managers, Skills Trainers and Nurses.
  • Document activities regarding counseling sessions, progress notes, and correspondence in a format prescribed by the Department of Veteran’s Services.
  • Assist with data collection for the purpose of reporting activity per the contract requirements.
  • Complete all documentation required for measuring quality and outcomes.
  • Participate in activities to assist and promote the Veteran’s Outreach program in gaining visibility and connections with community agencies, providers and the public.
  • Follow the principles of consumer directed care which focus on the unique and individual needs of each Veteran and their family.
  • Other duties as assigned.

Essential Functions/Qualifications:

  • Bachelor’s Degree in a human services area preferred; relevant work experience considered.
  • CPR and First Aid Certifications required.
  • Is a Veteran-- a person who served in the active military, naval, or air service, and who was discharged or released therefrom under conditions other than dishonorable.
  • Knowledge of Veterans Services, social service delivery systems and community resources.   
  • Awareness of public and Veterans benefits (preferred).
  • Valid Driver’s License; successfully passing Driving Record Check and meeting minimum insurance requirements.
