
Crisis Intervention Worker
Job description
The Crisis Worker determines the need for protective intervention and the delivery or arrangement of services for self-neglected elders. The worker carries a case load at program standard.
Responsibilities:
Accepts referrals from supervisor on appropriate cases requiring intensive and extensive casework.
Conducts an assessment of each client referred and establishes an appropriate service plan.
Maintains records regarding the case plan including physio-psycho-social characteristics and disposition of said clients.
Utilizes community professionals who can act as consultants in developing case plans for clients in crisis.
Provide backup and assistance to other Crisis Intervention and Protective Services staff when necessary, as directed by supervisor.
Prepares appropriate statistics and reports to document program activities.
Qualifications:
Bachelor’s degree in Social Work Human Services or related field preferred.
Motor vehicle and Valid Driver’s license; maintain minimum insurance liability on vehicle and complete motor vehicle driving record check
Knowledge of home and community-based service systems and how to access and arrange services.
Knowledge of MassHealth programs and public benefits.
